Community Health Assessment & Community Health Improvement Plan
A huge part of public health is planning for the future and anticipating the needs of the public.
Every three years, the Central District Health Department works with partners to assess the health and well-being of the community. This process is known as the Community Health Assessment, or the “CHA”. Through the CHA, we learn about things like the top illnesses in our counties, the biggest health concerns, what keeps people from getting the care they need, and more. We collect all this information and share it with our partners and with the public.
This information is then used to create a Community Health Improvement Plan, or a “CHIP”. Below you can find the complete CHA from 2024 & 2021 and our 2023-2025 CHIP.
